APPALACHIAN COMMUNITY TO BEGIN CHRONIC CARE CERTIFICATION FOR ALL CLINICIANS
Appalachian Community will be developing a new chronic disease care certification program for all of our nurses, therapists and home care aides.
Following Medicaid and Medicare restrictions, home care has long focused on the treatment of acute conditions, such as recovery from an accident or curable illness. Yet chronic diseases such as diabetes, coronary-pulmonary disease, and chronic heart failure are on the rise, and chronic disease is essentially at the heart of the vast majority of modern medical care.
Better monitoring and management of chronic diseases could prevent many hospitalizations, greater suffering, and higher costs. As new medical approaches to chronic disease are being developed, it has been recognized that chronic disease management must support the individual where they face their daily challenges – their home.
ACVNAHHS plans to be a leader in our community on this new care delivery model. Watch for more information as our chronic care program is implemented.
